Metro Vein Centers Monthly Cosmetic Spider Vein Treatment Giveaway — Official Terms & Conditions

1. Sponsor
- This giveaway (“Promotion”) is sponsored by Metro Vein Centers, with headquarters located at 7125 Orchard Lake Road Suite 120, West Bloomfield, MI 48322 (“Sponsor”).
2. Eligibility
- Open to legal residents of the United States, aged 18 years or older at the time of entry.
- Entrants must be able to independently travel to a Metro Vein Centers clinic to receive treatment.
- Limit one (1) entry per person and per email address. Duplicate entries will be disqualified.
- Employees of Metro Vein Centers, its affiliates, subsidiaries, advertising and promotion agencies, and immediate family members or those living in the same household are not eligible to participate.
3. Promotion Period
The specific monthly timeline of the Promotion (the “Promotion Period”) is provided in the communications about the Promotion, but the Promotion Period will begin at 12:00:01 a.m. Eastern Time (ET), as observed in Michigan, on the first calendar day of the specified month and ends at 11:59:59 p.m. ET on the last calendar day of the specified month. In the event any type of promotion is recurring from month-to-month, each Promotion shall stand alone, and entries from one Promotion will not carry over to any other Promotion. Also, there is no guarantee that any particular Promotion will be recurring or will continue to recur in the future; rather, Sponsor (as defined above) reserves the right to cancel any recurring promotions at any time and for any reason.
4. How to Participate
To enter, visit the giveaway landing page and complete the Cosmetic Spider Vein Treatment Giveaway Form with your full name, phone number, email address, and zip code. No purchase, follow, comment, or tag is required. By submitting the form, you agree to these Terms & Conditions.
5. Selection of Winners
- One winner will be selected at random from all eligible entries received.
- Winner will be contacted via email on or around the first business day of each month.
- Each winner must respond to the notification email within five (5) business days to claim their prize.
- If a winner does not respond within this timeframe, an alternate winner may be selected.
- If no eligible alternate winner responds within the applicable claim period, the prize will be forfeited and may not be re-awarded.
6. Prize
Each winner will receive one (1) $500 USD Visa gift card, intended for use toward a cosmetic spider vein treatment at a participating Metro Vein Centers location. If the winner decides not to proceed with treatment or their Metro Vein Centers Physician does not recommend treatment, the patient may use the gift card for anything.
- Treatment eligibility is subject to a medical evaluation by a Metro Vein Centers provider. Sponsor makes no guarantee that any specific treatment will be recommended or performed.
- Treatment (if applicable) must be scheduled and the gift card redeemed within eight (8) weeks of prize confirmation.
- Participating locations only. Winner is responsible for any costs exceeding the prize value.
7. Travel & Availability
Winners are responsible for their own transportation to and from the clinic.
Metro Vein Centers does not provide travel or lodging accommodations.
8. Privacy & Publicity
By entering, participants consent to the collection and use of personal information for the purpose of administering this Promotion and contacting winners.
As a condition of receiving the prize, winners agree to grant Metro Vein Centers the right to use their name, city, and general likeness for promotional purposes without additional compensation, unless prohibited by law. For more information, please review our Privacy Policy.
9. General Conditions
- Metro Vein Centers reserves the right to cancel, suspend, or modify the giveaway if fraud, technical failures, or other factors compromise the integrity of the Promotion.
- Metro Vein Centers reserves the right to disqualify any individual found tampering with the entry process or violating these Terms & Conditions.
- By participating, entrants release and hold harmless Metro Vein Centers and its employees, officers, and affiliates from any liability arising from participation or prize use.
10. Disputes
Except where prohibited, participants agree that any disputes related to this Promotion will be governed by the laws of the State of Michigan, without regard to conflict of law principles, and must be filed in the appropriate state or federal court within Michigan.
